Medical Insurance Plan Options

reliatech offers its employees the choice of three plans administered by Kaiser Permanente. Deductions are on a weekly basis. Eligibility for medical and dental benefits occur the first of the month following 60 days of employment. Health and dental are a package deal and are deducted from your paycheck on a weekly basis.

Dental PPO

The Company offers a dental plan under the administration of Delta Dental. The cost of the dental plan is included as part of your medical plan.

Vision

Vision is included in the medical plan.

Long-Term Disability

The employee, based on individual salary, pays the premium for long-term disability insurance.

Short-Term Disability

Employees are eligible the first of the month following 60 days of employment.

Term Life Insurance

Employees are eligible the first of the month following 60 days of employment.

Flex Spending Program

The Flex Spending Plan will be administered by BeniSource (for both Health Care Reimbursement and Dependent Care Reimbursement Accounts).

401k Savings Plan

Merrill Lynch administers the Savings Plan (401K) and all employees are eligible the first of the month after completing 90 days of employment.

Profit Sharing

The profit-sharing program is company contributed and based on a percentage of the employees base salary.

Health Advocate

Health Advocate, Inc. is an advocacy and assistance service company founded to specifically address many of the issues Americans encounter while accessing the healthcare and health insurance systems. Health Advocate's expertise and relationships in the healthcare industry combined with their in-depth knowledge of health and related insurance policies and procedures, give them the know-how to act on our associates' behalf. This service is provided at no cost to the employee, and can be used as soon as the first day of employment.

Unileave

UNILEAVE days are days which can be taken off from work and for which you will be paid as if you worked a standard eight-hour day at your normal rate. These days can be used for vacation (annual) leave, sick leave and holidays. Employees are eligible after completing 90-days of continuous employment.

Tuition Reimbursement

The company subsidizes tuition and books for employees taking job-related college courses. For tuition reimbursement to apply, classes must be pre-approved by Human Resources. When approved, reimbursement includes a percentage of the book cost and the tuition cost: 100 percent of the cost for an A, 80 percent for a B and 60 percent for a C. The company will reimburse for one course per semester and three courses per year. Employees are eligible after completing one year of continuous employment.

Employee Assistance Program

The company provides a counseling service to help employees with personal problems before they result in deterioration of health, family life and/or job performance. Employees are eligible for services after completing 90-days of continuous employment.

Credit Union Membership

Membership in the International Brotherhood of Electrical Workers (IBEW) Local 26 Credit Union is available for a $5.00 enrollment fee. Services include checking accounts, money market accounts, 24-hour audio response teller, ATM's and loans (i.e. signature, new and used auto).
